Meeting Criteria

Thrive Health Connection’s team of highly trained and skilled medical case managers delivers a comprehensive suite of individual-centered services designed to facilitate access to the quality clinical and supportive services needed to achieve and maintain optimal health and viral suppression. Our case managers are located at our main office as well as at several satellite locations. Our team is happy to meet you in the community or at your medical appointment. We offer bilingual (Spanish/English) case management services as well. Individuals eligible for case management services meet the following criteria:

Have an HIV/AIDS diagnosis

Have an income at or below 300% of the Federal Poverty Level.

Live in the 11-county area (Missouri: Cass, Clay, Clinton, Jackson, Lafayette, Platte, and Ray; Kansas: Johnson, Miami, Leavenworth and Wyandotte).

Basic Steps

If you’re interested in obtaining case management services, or if you’ve lost contact with your case manager, please contact Thrive Health Connection at 816-561-8784  or text or call 816-437-0524 and request to speak with the case manager of the day. The steps for obtaining services are below:

  • Complete a brief intake and screening form so that we can assess your need for services and determine the best location for case management – whether at our main office, one of our satellite sites or co-located at one of the HIV clinics or hospitals.
  • The intake and screening form is handed off to the Lead Case Manager and/or Case Management Supervisor for assignment.
  • Your assigned case manager will arrange for enrollment appointment and complete your intake assessment. You’ll work together to develop an Individualized Service Plan (ISP) to address your needs and overcome any barriers to care.
  • In order to expedite the process of enrolling in case management, please bring proof of your HIV status. We will also need documented proof of residency, income and insurance.
  • Individuals are referred to the appropriate services based on the needs identified during intake and enrollment. Our goal is to ensure you have access to medical care and are continuing your medical care treatment – but we recognize that this is not always the top priority. Our case managers work with you to overcome any number of health care barriers including housing, substance abuse, mental health issues and more. Our goal is to help you be successful in your medical care.
  • Once enrolled, individuals must re-verify eligibility every six (6) months and speak with their case manager to provide an update on your medical care.

Our primary case management goal is ensuring that everyone we serve is linked to and remain regularly engaged in medical care, an essential element of maintaining optimal health. Our medical case managers work with individuals to facilitate access to medical care. Our medical case managers and those they serve are partners – and that partnership is most successful when both are active participants.

In addition to medical case managers, Thrive Health Connection houses several specialty case managers:

  • Our Transitional Case Manager – one of only three in Missouri – who is specially trained to work with individuals who are currently or have recently been incarcerated. Our transitional case manager functions as a medical case manager but has a specialized focus on the incarcerated and the unique challenges that comes with a history of incarceration. The transitional case manager has access to additional service referrals for these individuals.
  • Our Linkage to Care Coordinators work with individuals who are newly diagnosed, who are new to the region, as well as individuals who have fallen out of care. Their primary goal is to help individuals engage with HIV medical care and to help provide support and education along the way. This is a short term program that typically lasts around 90-180 days. Once individuals successfully complete their time with linkage to care, they transfer to a medical case manager.   • Our Resource and Referral Coordinator works with individuals who do not have needs or barriers related to health outcomes. They work with individuals to provide support in healthcare navigation and link individuals to other Ryan White and community resources as needed.

We also house client services coordinator who works with case managers and clients customers to provide limited emergency assistance, limited bus passes, and access to the GoodStore for hygiene products for customers enrolled through Thrive Health Connection. Services are dependent on funding and not guaranteed.

Our client services coordinator in partnership with our case managers also helps link eligible customers to our 340B pharmacy, Coordinated Care Network. Individuals enrolled in case management through Thrive health Connection may be eligible to utilize our pharmacy if they have Medicare or private insurance.
